High-Holy-Days 5781

OOOPS, this form is a little more complex than previous years. Please read and fill out carefully

We hope you will join us for the High-Holy-Days!

We have designed several program options for you, and we are making every effort to ensure our services and facilities are safe and in full compliance with CDC recommendations.

If your health is compromised in any way, please remain at home. We will be sending out various materials to assist you in your home-experience.

As this year we have limited seats, we will process reservations based on a first-come-first-serve basis.

When joining at Shul, please pay attention to the following precautions:

- Everyone must wear a mask at all times
- We will measure your temperature before you enter the facilities
- Please wash and sanitize your hands frequently
- The chazzan will be surrounded by a plexiglass cover
- All seats will be distanced 8ft apart in all directions (family members may sit together)
- The facilities will be disinfected between each service
- Camp Metukah for the kids will be in small groups and led by staff who will be trained in COVID-19 safety measures

I acknowledge that the novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

I understand that despite that Beit David Highland Lakes Shul Inc. has put in place preventative measures to reduce the spread of COVID-19; however, Beit David Highland Lakes Shul Inc. cannot guarantee that I or members of my family will not become infected with COVID-19. Further, I understand that attending Beit David Highland Lakes Shul Inc. could increase the risk to me and/or my family of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the Beit David Highland Lakes Shul Inc. and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Beit David Highland Lakes Shul Inc. may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Beit David Highland Lakes Shul Inc. employees, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my attendance at Beit David Highland Lakes Shul Inc. or participation in Beit David Highland Lakes Shul Inc. programming ("Claims"). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Beit David Highland Lakes Shul Inc., its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Beit David Highland Lakes Shul Inc., its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any of Beit David Highland Lakes Shul Inc.'s programs.
